For years, the public has heard that a glass of wine a day may offer some health benefits. But a sweeping new study suggests that even modest drinking could quietly erode cognitive health over time, adding weight to growing evidence that there may be no truly safe amount of alcohol when it comes to dementia.
Published Tuesday in the journal BMJ Evidence-Based Medicine, the research reexamines decades of assumptions about alcohol’s neurological impact and raises tough questions about what we think of as “moderate” drinking.
Led by Anya Topiwala, a senior clinical researcher in psychiatry at the University of Oxford, the study analyzed both genetic and observational data to understand how alcohol affects dementia risk. Researchers drew on data from almost 560,000 participants in the UK Biobank, a massive long-term health study that tracks people in England, Scotland, and Wales, and from the US Million Veteran Program, which includes people of European, African, and Latin American ancestry.
Using a method called Mendelian randomization, the team assessed whether genes linked to alcohol consumption might directly influence the likelihood of developing dementia. Unlike traditional observational studies, which rely on self-reporting, Mendelian randomization minimizes errors caused by memory lapses or reverse causation (for instance, early dementia leading someone to drink differently).
This type of genetic analysis, she explained, is especially powerful because it can capture the cumulative impact of alcohol across a lifetime and not just what people drink in middle or old age.
For decades, research on drinking and brain health painted a confusing picture. Some studies suggested that light or moderate drinking, often defined as fewer than seven drinks a week, might be more “neuroprotective” than complete abstinence. But these studies had major flaws.
“The genetic analysis results showed that even small amounts of alcohol could increase dementia risk. This is the largest study on the topic, and the combination of observational and genetic analyses was key.”
They typically focused on older adults and often lumped together former drinkers (some of whom quit due to health issues) with lifelong non-drinkers. That overlap may have skewed results, making it appear as if abstainers had worse outcomes when, in reality, they were already at higher risk due to pre-existing health conditions.
By disentangling genetics from lifestyle factors, the new study sidesteps many of those pitfalls. And its conclusions are sobering.
After analyzing genetic data from 2.4 million people across 45 dementia studies, researchers found a clear pattern: dementia risk rises steadily as alcohol consumption increases.
“There is a 15% higher dementia risk for three drinks per week compared to one drink per week across life,” Topiwala said.
Even more striking, a doubling of genetic risk for alcohol dependency corresponded with a 16% increase in dementia risk.
These findings indicate a linear relationship, meaning that any increase in lifetime alcohol use, no matter how small, is associated with a higher risk.
The study’s implications have left neurologists and dementia researchers taking note.
“This is a fairly complicated study that provides some, but not definitive, evidence that alcohol can harm the brain regardless of how much is consumed,” said Dr. Richard Isaacson, director of research at the Institute for Neurodegenerative Diseases in Florida.
Isaacson, who studies cognitive decline in people genetically predisposed to Alzheimer’s, said the findings align with what he tells his own patients.
“In my clinic, we tell people with the APOE4 genetic variant, which is the most common genetic risk factor for Alzheimer’s, that drinking zero alcohol is best based on the available evidence.”
However, he added, context still matters. For people with less genetic risk for Alzheimer’s, it often depends on when and how people drink, Isaacson said. Two drinks before bed on an empty stomach several nights a week will have a more harmful effect on brain health compared with one drink a few times a week with an early dinner.
The study didn’t rely on genetics alone. Researchers also examined real-world drinking patterns from participants who self-reported their habits.
“In the self-reporting study, people who reported consuming small amounts of alcohol (less than 7 drinks per week) had lower risk than heavy drinkers (more than 40 drinks per week),” said Tara Spires-Jones, professor of neurodegeneration at the University of Edinburgh and director of the Centre for Discovery Brain Sciences.
“Interestingly, in this part of the study, non-drinkers and people who reported never drinking actually had similar risk of dementia to people who drank heavily,” Spires-Jones added.
That paradox has long puzzled scientists. One likely explanation is that some non-drinkers may have stopped drinking due to underlying health issues, skewing comparisons. When combined with the genetic data, however, the overall pattern becomes clearer: alcohol’s effect on the brain appears consistently harmful, even if the degree varies.
While the new study can’t definitively prove that alcohol causes dementia, Spires-Jones said it supports a large body of research pointing in that direction.
“This adds to a large amount of similar data showing associations between alcohol intake and increased dementia risk, and fundamental neuroscience work has shown that alcohol is directly toxic to neurons in the brain.”
Alcohol exposure has been shown to shrink brain volume, damage white matter (which connects brain regions), and disrupt neurotransmitter systems involved in memory and decision-making. Over years or decades, that cumulative effect may accelerate neurodegenerative processes already at play in aging or genetic susceptibility.
Dementia remains one of the world’s most pressing health concerns, affecting an estimated 55 million people globally — a number projected to double every 20 years. Alzheimer’s disease, the most common form, is influenced by both genetics and lifestyle factors, from diet and exercise to sleep and alcohol intake.
Public health officials have already begun rethinking how they communicate drinking risks. Countries such as Canada and Australia have revised national guidelines in recent years to warn that no amount of alcohol is truly risk-free.
The new findings may push that conversation further.
Topiwala hopes the study will encourage people to think critically about what “moderation” really means. It’s not just heavy drinking that matters; Even small, regular amounts could add up over time.
From toasts at weddings to happy hours after work, alcohol occupies a powerful social and emotional space in modern life. Yet studies like this one are forcing a reckoning with how deeply normalized drinking continues to be.
Moderation, once considered the gold standard of healthy living, may not be so harmless after all. This study suggests we may need to redefine what’s especially for people with any level of genetic risk.
